U roku od jednog dana nakon pozitivnog testa na covid-19 u lipnju ove godine, Amerikanka Miranda Kelly i njezin suprug ozbiljno su se zabrinuli za svoje zdravlje.

Supružnici su se odmah složili nakon što su dobili dijagnozu da se uključe u kliničko ispitivanje u obližnjem Centru za istraživanje raka Freda Hutchinsona, koje je dio međunarodnih nastojanja da se testira antivirusno liječenje koje bi moglo zaustaviti covid-19 odmah na početku, prenosi portal RTL televizije.

Već od sljedećeg dana njih dvoje uzimalo je po četiri tablete dva puta dnevno. Iako im nije rečeno jesu li primili aktivni lijek ili placebo, u roku od tjedan dana, simptomi su se počeli povlačiti. U roku od dva tjedna su se oporavili.

"Ne znam jesmo li dobili lijek, ali osjećam se kao da jesmo. Osjećala sam se kao da je oporavak bio vrlo brz, unatoč prethodnim zdravstvenim tegobama", rekla je Miranda Kelly.

Par je sudjelovao u razvoju onoga što bi mogla biti nova šansa u borbi protiv covida - kratkoročni režim dnevnih tableta koje mogu napasti virus ubrzo nakon dijagnoze i vjerojatno spriječiti razvoj simptoma, piše NBC News.

"Oralni antivirusni lijekovi imaju potencijal ne samo skratiti trajanje sindroma covida-19, već i ograničiti prijenos na ljude u kućanstvu bolesnika", rekao je Timothy Sheahan, virolog sa Sveučilišta North Carolina-Chapel Hill koji sudjeluje u osmišljavanju ove terapije.

Antivirusni lijekovi već se koriste za druge virusne infekcije, kao što su hepatitis C i HIV. Jedan od najpoznatijih je Tamiflu, široko korištena tableta koja može skratiti trajanje gripe i smanjiti rizik od hospitalizacije ako se brzo primijeni.

Ovi lijekovi, razvijeni za liječenje i sprječavanje virusnih infekcija kod ljudi i životinja, djeluju različito, ovisno o vrsti.

Najmanje tri obećavajuća antivirusna lijeka za covid testiraju se trenutačno u kliničkim ispitivanjima, a rezultati se očekuju već u kasnu jesen ili zimu.

Glavni konkurent je lijek kompanija Merck&Co. i Ridgeback Biotherapeuticsa nazvan molnupiravir. To je proizvod koji se testirao kod bračnog para Kelly u Seattleu. Dva druga su kandidati iz Pfizera te antivirusni lijek kojeg razvijaju Roche i Atea Pharmaceuticals.

Lijekovi ometaju sposobnost virusa da se replicira u ljudskim stanicama. U slučaju djelovanja molnupiravira, enzim koji kopira virusni genetski materijal prisiljen je napraviti toliko pogrešaka da se virus više ne može reproducirati.

Tako se smanjuje virusno opterećenje, skraćuje vrijeme infekcije i sprječava opasan imunološki odgovor koji može uzrokovati ozbiljne bolesti ili smrt. Do sada je samo jedan antivirusni lijek, remdesivir, odobren za liječenje covida. Daje se pacijentima koji su hospitalizirani i nije namijenjen širokoj upotrebi. 

Pfizer je 1. rujna pokrenuo kombinirano ispitivanje svog proizvoda u drugoj i trećoj fazi, a dužnosnici Atee rekli su da očekuju rezultate ispitivanja u drugoj i trećoj fazi krajem ove godine.

Budu li rezultati pozitivni i za bilo koji proizvod bude odobrena hitna uporaba, distribucija bi mogla početi vrlo brzo. To bi značilo da će milijuni ljudi uskoro imati pristup oralno primijenjenim lijekovima, idealno jednoj tableti dnevno, koja bi se mogla uzimati pet do 10 dana nakon potvrde infekcije covidom.

Nekada zapostavljeni zbog nedostatka interesa, oralni antivirusni lijekovi za liječenje infekcija koronavirusom sada su predmet žestoke konkurencije i financiranja. Bidenova je administracija naglasila da će uložiti 3,2 milijarde dolara u Antivirusni pandemijski program, čiji je cilj razvoj antivirusnih sredstava za covid i druge virusne krize.

Široko dostupni antivirusni lijekovi pridružili bi se monoklonskim terapijama protutijelima koja su se već koristila za liječenje i sprječavanje ozbiljnih bolesti i hospitalizacija uzrokovanih covidom. Monoklonska protutijela proizvedena u laboratoriju, koja oponašaju prirodni odgovor tijela na infekciju, lakše su razvijena, ali se moraju davati prvenstveno intravenoznim infuzijama. 

"Poput monoklonskih protutijela, antivirusne tablete ne bi bile zamjena za cijepljenje, One bi bile još jedan alat u borbi protiv covida. Lijepo je imati drugu opciju", rekao je znanstvenik Griffin.

Sudionici moraju biti necijepljeni i uključeni u ispitivanje u roku od pet dana od pozitivnih testova na covid. Svakog dana pripravnici nazivaju 100 novooboljelih osoba u području Seattlea, ali većina odbija sudjelovati.

"Općenito govoreći, postoji puno nepovjerenja u znanstveni proces. A neki ljudi pripravnicima govore ružne stvari", objasnio je Duke za NBC News.

Ako se antivirusne tablete pokažu učinkovitima, sljedeći će izazov biti pojačanje distribucijskog sustava koji ih može podijeliti ljudima čim budu pozitivni. Dužnosnici Mercka predvidjeli su da bi kompanija do kraja godine mogla proizvesti više od 10 milijuna terapijskih kompleta. Atea i Pfizer nisu objavili slične procjene, prenosi portal RTL televizije.

U roku od jednog dana nakon pozitivnog testa na covid-19 u lipnju ove godine, Amerikanka Miranda Kelly i njezin suprug ozbiljno su se zabrinuli za svoje zdravlje.

Supružnici su se odmah složili nakon što su dobili dijagnozu da se uključe u kliničko ispitivanje u obližnjem Centru za istraživanje raka Freda Hutchinsona, koje je dio međunarodnih nastojanja da se testira antivirusno liječenje koje bi moglo zaustaviti covid-19 odmah na početku, prenosi portal RTL televizije.

Već od sljedećeg dana njih dvoje uzimalo je po četiri tablete dva puta dnevno. Iako im nije rečeno jesu li primili aktivni lijek ili placebo, u roku od tjedan dana, simptomi su se počeli povlačiti. U roku od dva tjedna su se oporavili.

"Ne znam jesmo li dobili lijek, ali osjećam se kao da jesmo. Osjećala sam se kao da je oporavak bio vrlo brz, unatoč prethodnim zdravstvenim tegobama", rekla je Miranda Kelly.

Par je sudjelovao u razvoju onoga što bi mogla biti nova šansa u borbi protiv covida - kratkoročni režim dnevnih tableta koje mogu napasti virus ubrzo nakon dijagnoze i vjerojatno spriječiti razvoj simptoma, piše NBC News.

"Oralni antivirusni lijekovi imaju potencijal ne samo skratiti trajanje sindroma covida-19, već i ograničiti prijenos na ljude u kućanstvu bolesnika", rekao je Timothy Sheahan, virolog sa Sveučilišta North Carolina-Chapel Hill koji sudjeluje u osmišljavanju ove terapije.

Antivirusni lijekovi već se koriste za druge virusne infekcije, kao što su hepatitis C i HIV. Jedan od najpoznatijih je Tamiflu, široko korištena tableta koja može skratiti trajanje gripe i smanjiti rizik od hospitalizacije ako se brzo primijeni.

Ovi lijekovi, razvijeni za liječenje i sprječavanje virusnih infekcija kod ljudi i životinja, djeluju različito, ovisno o vrsti.

Najmanje tri obećavajuća antivirusna lijeka za covid testiraju se trenutačno u kliničkim ispitivanjima, a rezultati se očekuju već u kasnu jesen ili zimu.

Glavni konkurent je lijek kompanija Merck&Co. i Ridgeback Biotherapeuticsa nazvan molnupiravir. To je proizvod koji se testirao kod bračnog para Kelly u Seattleu. Dva druga su kandidati iz Pfizera te antivirusni lijek kojeg razvijaju Roche i Atea Pharmaceuticals.

Lijekovi ometaju sposobnost virusa da se replicira u ljudskim stanicama. U slučaju djelovanja molnupiravira, enzim koji kopira virusni genetski materijal prisiljen je napraviti toliko pogrešaka da se virus više ne može reproducirati.

Tako se smanjuje virusno opterećenje, skraćuje vrijeme infekcije i sprječava opasan imunološki odgovor koji može uzrokovati ozbiljne bolesti ili smrt. Do sada je samo jedan antivirusni lijek, remdesivir, odobren za liječenje covida. Daje se pacijentima koji su hospitalizirani i nije namijenjen širokoj upotrebi. 

Pfizer je 1. rujna pokrenuo kombinirano ispitivanje svog proizvoda u drugoj i trećoj fazi, a dužnosnici Atee rekli su da očekuju rezultate ispitivanja u drugoj i trećoj fazi krajem ove godine.

Budu li rezultati pozitivni i za bilo koji proizvod bude odobrena hitna uporaba, distribucija bi mogla početi vrlo brzo. To bi značilo da će milijuni ljudi uskoro imati pristup oralno primijenjenim lijekovima, idealno jednoj tableti dnevno, koja bi se mogla uzimati pet do 10 dana nakon potvrde infekcije covidom.

Nekada zapostavljeni zbog nedostatka interesa, oralni antivirusni lijekovi za liječenje infekcija koronavirusom sada su predmet žestoke konkurencije i financiranja. Bidenova je administracija naglasila da će uložiti 3,2 milijarde dolara u Antivirusni pandemijski program, čiji je cilj razvoj antivirusnih sredstava za covid i druge virusne krize.

Široko dostupni antivirusni lijekovi pridružili bi se monoklonskim terapijama protutijelima koja su se već koristila za liječenje i sprječavanje ozbiljnih bolesti i hospitalizacija uzrokovanih covidom. Monoklonska protutijela proizvedena u laboratoriju, koja oponašaju prirodni odgovor tijela na infekciju, lakše su razvijena, ali se moraju davati prvenstveno intravenoznim infuzijama. 

"Poput monoklonskih protutijela, antivirusne tablete ne bi bile zamjena za cijepljenje, One bi bile još jedan alat u borbi protiv covida. Lijepo je imati drugu opciju", rekao je znanstvenik Griffin.

Sudionici moraju biti necijepljeni i uključeni u ispitivanje u roku od pet dana od pozitivnih testova na covid. Svakog dana pripravnici nazivaju 100 novooboljelih osoba u području Seattlea, ali većina odbija sudjelovati.

"Općenito govoreći, postoji puno nepovjerenja u znanstveni proces. A neki ljudi pripravnicima govore ružne stvari", objasnio je Duke za NBC News.

Ako se antivirusne tablete pokažu učinkovitima, sljedeći će izazov biti pojačanje distribucijskog sustava koji ih može podijeliti ljudima čim budu pozitivni. Dužnosnici Mercka predvidjeli su da bi kompanija do kraja godine mogla proizvesti više od 10 milijuna terapijskih kompleta. Atea i Pfizer nisu objavili slične procjene, prenosi portal RTL televizije.
